New Clients

Deciding to get counseling is a big deal. It’s a weighty decision and choosing where to start and who to work with is both personal and consequential. We want to help you make the right choice for you. Here’s what you can expect when you call Creekside: Our support staff are available to help you schedule an initial appointment and answer questions you might have. They’ll work closely with you in selecting a clinician who is a good fit for you. If you are calling regarding pornography or sex addiction, please let the front desk person know so you can be directed to the counselors who are specifically trained to meet those needs.

For your first appointment you’ll be asked to come 20 minutes early to complete necessary paperwork. Should you prefer, you may choose to access a portion of our paperwork below. Simply print, complete, and bring your paperwork  with you to your initial session. You’ll then need to arrive 10 minutes before your scheduled appointment. If you are scheduling for counseling only, the medication management paperwork is not needed.

For appointments with our nurse practitioner, all paperwork must be completed and returned to our office before an appointment is scheduled.
